This plan has lower premiums per month and has a higher deductible than the Keycare Choice PPO plan.

A PPO, or Preferred Provider Organization doesn’t require you to go to a primary care doctor before visiting a specialist.

Preventive Care: Pays 100% in-network; 50% out of network after deductible.

Deductibles for Key Care STANDARD PPO plan: 

In Network: $1,500 Individual/$3,000 Family.

Out of Network: $3,000 Individual/$6,000 Family.

Out-of-Pocket Max: In-Network: $5,000 Individual/$10,000 Family. Out of Network: $10,000 Individual/$20,000 Family.

3) Keycare High Deductible Health Plan (HDHP):

An HDHP or High Deductible Health Plan, has higher deductibles, but lower monthly premiums. Like a PPO, you aren’t required to see a primary care doctor, and they generally cost less per month.
However, you must cover the costs of medical expenses until you meet your deductible.

 

An HSA (Health Savings Account) is often attached to a HDHP. HSAs allow you to set aside pre-tax dollars for out-of-pocket health expenses, which helps to cover the cost of a high deductible.

Preventive Care: Each of the Anthem medical plans cover in-network preventive care at 100%. This includes routine screenings and checkups, as well as counseling to prevent illness, disease, or other health problems.

Your local Select Network is based on the state in which you reside.

  • Northern Virginia*: Blue Choice Advantage Open Access

These local doctors and hospitals have agreed to a discounted rate. So, when you go to them, those savings will pass on to you. Most local hospitals and providers participate in the local network, which means you will have several high-quality and cost-effective providers to choose from. When you travel outside of your home state, and therefore out of the Select Network area, you will still have access to the BlueCard PPO network.

Listed below are the per-pay-period (26 pay periods per year) deductions (costs) for medical insurance. The amount you pay for coverage is deducted from your paycheck on a pre-tax basis, which means you don’t pay taxes on the amount you pay for coverage.
SPOUSAL SURCHARGE: A spousal surcharge of $60 per month will apply to the “employee + spouse” and “employee + family” coverage levels if your spouse is offered medical insurance through their employer and enrolls in a Centurion medical plan.
